We operate the Formvoice service, a feedback collection platform that forwards submissions directly to form owners.
Formvoice provides:
Free Tier: Up to 1,000 submissions per form or per user account
Future Paid Plans: Will be available for higher usage (one-time payment, no subscriptions)
Fair Use: We reserve the right to limit excessive usage that impacts service quality for other users
We take best effort precautions against spam, bots, and bad actors via Cloudflare services, including Turnstile captcha checks for every submission and more.
You agree to:
Content Responsibility: Submitters are responsible for the content they submit. Form owners are not liable for third-party submissions they receive.
Prohibited Uses:
Service "As Is": Formvoice is provided without warranties of any kind, express or implied.
Limitation of Liability: To the maximum extent permitted by Dutch law, we are not liable for:
Maximum Liability: Our total liability for paid services shall not exceed the amount you've paid us in the past 12 months. For free users, our liability is limited to the minimum allowed under Dutch law.
Your Content: You retain all rights to content submitted through your forms, in so far as that is not retained by the submitter
Our Service: Formvoice and its features remain our intellectual property
License to Us: You grant us a limited license to process and forward your data as necessary to provide the service
Service Changes: We may modify features at any time. For significant changes that materially affect your use, we'll provide notice via the admin page. We aim to minimize negative impacts on existing functionality
Your Termination: You can stop using Formvoice at any time

Governing Law: These Terms are governed by Dutch law

Formvoice is intended for use by adults (18+) creating feedback forms. Form creators must ensure their forms comply with applicable laws when collecting data from minors. We do not verify the age of form submitters, as this is the responsibility of the form creator.
We don't use automated decision-making or profiling that produces legal effects concerning you.
